If you agree to it, please sign it and then proceed to the Application, Rules and Guidelines, House Rules and Questionnaire forms leaving NO blanks. Be sure to include a contact number that we can call to arrange a phone interview between you and the director. All forms may be faxed or mailed. The $25.00 non-refundable application fee can be sent by mail.

When all are received, the application will be processed and arrangements for the phone interview will be made.

If you are incarcerated, provide the number for the person who will arrange the phone interview. A collect call will be accepted by New Beginnings as long as it has been set up between the contact person and New Beginnings Ministry.
